Student Health Information
At the beginning of each school year, parents or guardians complete and return important health information about their child which is kept by the school nurse. In the event of an emergency, this information is used to ensure proper care for your child. The Health History form also requests contact information so that the school nurse can reach parents/guardians during the day, if needed. Please notify us of any changes in your child’s health or in your contact information as it occurs during the year. A confidential health record which includes medical history, immunizations, screening results and any physical examination reports, is kept by the nurse for each student throughout his/her school career.
